Below is an overview of how the FMHC onboarding journey typically works, and how our team supports you at each stage.
1. Understanding Eligibility and Getting Started
To begin FMHC, your loved one must undergo an assessment with Ontario Health atHome. A Care Coordinator evaluates care needs and develops a Plan of Service.
If you believe FMHC is the right fit, you simply inform your Care Coordinator that you wish to receive care through the FMHC program to ensure more personalized, flexible, and family‑directed support.
2. Referral to the FMHC Program
Once you express interest, your Ontario Health atHome Care Coordinator will submit a referral to the FMHC program. This initiates the formal onboarding process.
3. Initial FMHC Program Meeting
An FMHC Care Coordinator will contact you or your Substitute Decision Maker (SDM) to schedule an introductory meeting. During this meeting, you will receive a Patient Onboarding Checklist, outlining all documents required for FMHC approval. Families are typically given 60 days to complete and submit these documents.
4. Support from Lifetime CarePros
